Defect:

Axles were inadequately heat treated. consequence of defect: under ordinary use, axles may bend, which may cause separation of the wheel assembly from the axle or the vehicle. this could result in an accident.

Correction:

Vehicles should not be operated until axles have been replaced with new original equipment specification axles.
