Defect:

The keeper springs (from the snap hooks) can become disengaged from the hook/keeper, allowing inadvertent release of the snap hook.

Consequence:

This would not adequately restrain a wheel chair passenger in the event of a sudden stop or accident and could result in a personal injury.

Correction:

The defective hook will be replaced with a new design hook which will not allow the spring to escape from the hook. these hooks have been provided to distributors.
