Defect:
The automatic throttle chain can catch on the throttle linkage, causing the throttle to remain open.
Consequence:
The operator would have difficulty stopping the vehicle.
Correction:
Retrofit kits will be provided for all affected vehicles including procedures to obtain local service.

Defect:
The ambulances were built with "l" shaped entry door assist rails. the rails are located such that when used with an open hand, the fingers can be caught in the door while closing.
Consequence:
Personal injury can occur when the door is closed on the fingers.
Correction:
Ambulance operators will be provided with guards and installation instructions or if the operator prefers, wheeled coach will install the guards for them.
