Incorrect lug bolts may have been used in the manufacturing of the spare wheel carrier. these lug bolts may break allowing the spare wheel to fall off. consequence of defect: breakage of the lug bolts allowing the spare wheel to fall to the road way will create a road hazard for other vehicles.

Correction:

Install proper size wheel lug bolts.
